Case Management

 

Case Management Case management has developed into a major function for The Arc of Greensboro, especially since state and local government transferred this responsibility to private, community-based nonprofit organizations. North Carolina's Community Alternatives Program (CAP) for Persons with Mental Retardation / Developmental Disabilities (MR/DD) – funded with Medicaid dollars – offers access to a wide range of home and community-based services for individuals with intellectual disabilities, especially those who would otherwise require care in an intermediate care facility (ICF). In accordance with government regulations, CAP service plans must be coordinated by well-qualified case management organizations in collaboration with the client and their family.The Arc's team of credentialed case managers and community resource specialists provided more than 6,000 hours of case management during 2007. The essential components of effective case management include:

  • Assessing the client's needs
  • Developing a person-centered care plan with the client and, in most cases, the client's family
  • Negotiating the client's access to needed services and supports
  • Monitoring progress in the care plan.

While professional training is essential, case managers need to be particularly sensitive to the social, cultural and economic factors that can impact theuse of services.
